The Fairfax County Long Term Care Coordinating Council

Fairfax County Department of Family Services’ Disability Services Planning and Development

ADD YOUR PROGRAM OR ORGANIZATION TO THE DISABILITYNAVIGATOR DATABASE

Do you provide a program or service in Northern Virginia or the surrounding area that serves individuals with disabilities and/or their families?

Listing your program on the disAbilityNavigator website will raise awareness about your service and ensure that the people who need it the most find it easily.

How do you add your program?

1. Fill out a form (PDF) with your information and fax it. (follow instructions on form)

References may be made to SeniorNavigator in the form but rest assured that your information will appear on disAbilityNavigator when it is launched. The SeniorNavigator database is being used to manage information at the present time.
