Who We Are
Colonial Behavioral Health offers Adult Case Management which helps individuals with developmental disabilities access supports and services critical to meeting basic needs. improving quality of life, and promoting self-empowerment. Ongoing assessment and person-centered planning are central to Case Management. Services are provided to support the individual's plan including:
- Assessment
- Service planning
- Service coordination
- Help to obtain/maintaining services
- Connection to providers
- Implementation of the person-centered plan
- Supportive counseling
- Monitoring
- Hospital discharge planning.
