Who We Are
Quality Connect, LLC is an agency that will provide structured support, life skill development, and case management assistance to adults with a mental health diagnosis who demonstrates a need for greater stability and personal management within their home and community. Services will be provided following a determination of eligibility and the completion of an assessment to identify the individual's needs. The mission is to provide comprehensive, integrated mental health, substance abuse, community skill-building, and developmental services for those who are disability-diagnosed, that promote the health and quality of their community members.

Residency may vary. Ex. Private residence, adult-assistance living, and homeless individuals are all accepted.
Other Eligibility Criteria
Individuals ages 18-75 who demonstrate difficulty with interpersonal relationships, interventions, cognitive ability, and basic living skills.
Intake Process
Access services through either a referral system in which a physician, therapist, or significant mental health professional could suggest the service for them. The individual may seek services independently and go forth with the intake process. Networking via community events, website sharing, promotion through social media, or building rapport with similar mental health agencies in close proximity such as day supports, therapists, local hospitals, assisted living programs, etc.
