Short Term Personal Home Care

Senior Connections, Short Term Home Care program provides scholarships or financial assistance on a sliding fee scale for adults 60 years of age and older in need of personal care (i.e., bathing, dressing, toileting, eating, or transferring).  Senior Connections contracts with local home care agencies to provide this service. Senior Connections is not home care or home health agency. 



This program supports individuals recovering from illness; applying for Medicaid and Medicaid CCC Plus; seeking facility placement; or, are in the process of making long-term care arrangements. Grants may also be used for respite for family caregivers. A maximum of 80 hours of in-home personal care may be approved per family per year.  

All individuals requesting Short Term Home Care will be assessed through our Care Coordination Program for eligibility and needs for other long-term care supports.
