Who We Are
The Life Coach Program connects adults, teens, and children with chronic conditions who use the emergency department as their medical home to a primary care physician. This is done by the life coach scheduling the appointment for the client, calling them to remind them of the upcoming appointment and then following up with the client to make sure they made the appointment and that everything went well. The Life Coach will also help to connect clients to community resources as needed.

Catholic Charities’ Life Coach Program is available to emergency room patients at Bon Secours Maryview Emergency Department in Portsmouth, Bon Secours Mary Immaculate Emergency Department in Newport News, Sentara OBICI Emergency Department in Suffolk, and Chesapeake Regional Medical Center.
