Case Management, Brain Injury
Programs that develop plans for the evaluation, treatment and/or care of individuals who, because of brain injury need assistance in planning and arranging for services; which assess the individual's needs; coordinate the delivery of needed services; ensure that services are obtained in accordance with the case plan; and follow up and monitor progress to ensure that services are having a beneficial impact on the individual.

Community Based Clubhouse and Day Programs, Acquired Brain Injury
“Stand alone” programs offering work-related and social activities in a supportive environment. Provide limited case management services that include the development of individualized plans of services with personal goals; provides educational, vocational, and therapeutic social environments for survivors of acquired brain injury.

Companion Services
Programs and/or services that provide general companionship, friendship, socialization and minimal assistance with daily activities.
